Clinical Study The Incidence and Prognostic Value of Hypochloremia in Critically Ill Patients
نویسندگان
چکیده
Little is known on the clinical effects of chloride on critically ill patients. We conducted this retrospective, observational study in 488 critically ill patients to investigate the incidence of chloride abnormalities, effects of hypochloremia in acid-base disorders, and association between chloride and clinical outcome. The study involved retrieval of arterial blood gas analyses, biochemical and demographical data from electrical records as well as quantitative acid-base analyses. For statistical analysis, the patients were stratified into three groups according to their chloride level (normal range: 98–106mEq/L). The distribution of chloride levels was hyperchloremia 16.6%, normochloremia 74.6%, and hypochloremia 8.8%. The hypochloremic group was significantly alkalemic (P < 0.0001) and has significantly higher apparent strong ion difference (SIDa) (P < 0.0001) compared to the two other groups. The hypochloremic group had significantly longer stays in the ICU and hospital (P < 0.0001) with higher mortality (P < 0.0001). However, multiple regression analysis showed that chloride was not an independent factor of poorer outcome. In conclusion, the acid-base characteristics of the hypochloremic patients were alkalemia coexisting with higher SIDa. And although it was not an independent prognostic factor, hypochloremia was related to poorer outcome in critically ill settings.

متن کاملPotential Role of Exogenous Melatonin Supplement in Delirium Prevention in Critically Ill Patients: A Double-Blind Randomized Pilot Study
Critically ill patients often suffer from disturbance of sleep-wake cycle and consequently delirium development, in intensive care units (ICU). In this study, we aimed to evaluate the effect of exogenous melatonin on delirium development and its related adverse sequelae in the subgroup of medical and surgical ICU patients. We performed a double-blind placebo-controlled randomized pilot study i...

متن کاملRisk Factors and Prognostic Factors of Acute Renal Failure in Patients Admitted to an Intensive Care Unit, Tehran-Iran
Acute renal failure (ARF) is defined as a sudden and continuous decrease of glomerular function associated with azotemia, and may be followed by decreased urinary output. There is a high incidence of ARF in ICU patients with a high mortality rate. Many factors can promote ARF development or influence its outcome. This study was done to assess the incidence, risk factors, outcome and treatme...
متن کاملComparison of the effect of Coenzyme Q10 and N-acetyl cysteine in prevention of acute kidney disease in critically ill patients admitted to Pediatric Intensive Care Unit
Background: Acute renal injury (AKI) is one of the most common causes of mortality in children admitted to intensive care units (PICU). Here, we investigated the preventive effect of N-acetylcysteine (NAC) and Coenzyme Q10 on AKI among children admitted to PICU. Methods: This double-blind randomized clinical trial study was done on 78 children admitted to PICU in 2018. Patients were randomly...
